Clinical Observation of Congenital Hypertrophic Pyloric Stenosis

Abstract
Nine cases of congenital hypertrophic pyloric stenosis which is one of the commonest surgical disease in newborn period were treated surgically at the Han Yang University Hospital during the past 3 years from May, 1972 to July, 1975.
The acquired results were as follows:
1. The overall incidence of congenital hypertrophic pyloric stenosis was 4.5% of all pediatric surgical patients admitted to Dpt. of General Surgery of H.Y.U.H. and the ratio of male to female was 2 : 1.
2. Seven in nine cases were underdeveloped and eight in nine cases were breast-fed.
3. Moderately and severely dehydrated six cases had anemia and hypochloremic metabolic alkalosis on admission.
4. Projectile vomiting were noted in all cases and the olive shaped masses were palpated in eight cases.
5. Generalized jaundice was found in one case and it was disappeared completely on the postoperative 3rd. day.
6. Upper gastrointestinal series was done in all cases and the X-ray films showed the characteristic signs of congenital hypertrophic pyloric stenosis; such as string sign, delayed passage of pyloric canal, mushroom appearance and gastric dilatation.
7. Fredet-Ramstedt operation was performed in all cases and excellent results were obtained in eight cases but one case was expired due to the previous respiratory distress syndrome on the postoperative 12 hours.
8. Recurrence of vomiting following surgery was observed in three cases during first 2-3 feedings.
